Real Talk Counseling now offers Substance Abuse Professional (SAP) Services for individuals who are required to complete the Department of Transportation (DOT) return-to-duty process following a drug or alcohol violation.
SAP services are federally regulated and designed to protect public safety while supporting individuals in safely returning to safety-sensitive work.
A Substance Abuse Professional (SAP) is a qualified clinician authorized under 49 CFR Part 40 to evaluate employees who have violated DOT drug and alcohol regulations. SAPs do not advocate for the employer or the employee β their role is to ensure compliance with federal safety standards.
SAP services are required for individuals working in DOT-regulated industries, including:
Commercial Trucking (FMCSA)
Aviation
Railroads
Transit
Pipeline
Maritime
DOT-Compliant Initial SAP Evaluation
Clinical Assessment & Determination of Treatment Level
Written Treatment and Education Recommendations
Monitoring of Treatment Compliance
Follow-Up SAP Evaluation
Return-to-Duty & Follow-Up Testing Recommendations (per DOT guidelines)
Communication with Employers, DERs, and MROs (with proper releases)

SAP evaluations do not guarantee a return-to-duty decision
SAPs do not perform drug or alcohol testing
SAP services are separate from treatment services
All determinations are based on DOT regulations and clinical judgment
If treatment or education is required:
Clients are free to choose their own provider
Real Talk Counseling may provide outpatient services when clinically appropriate
We also refer to external IOP or inpatient providers if required or recommended
All referrals are made in accordance with DOT and state guidelines
SAP services are required for individuals who:
Failed or refused a DOT drug or alcohol test
Tested positive for prohibited substances
Violated DOT drug and alcohol regulations
Are seeking reinstatement in a safety-sensitive position
Initial Evaluation
A comprehensive assessment to determine treatment or education requirements.
Treatment / Education Phase
Completion of required services through an approved provider.
Follow-Up Evaluation
Confirmation that all recommendations have been successfully completed.
Return-to-Duty Recommendation
Documentation provided to the employer or designated representative.
SAP services are self-pay and not covered by insurance due to federal regulations.
Payment is required prior to service.
